Form Designer© is a
robust development tool which
can be used by the novice or
professional alike. It
allows the physician or
researcher to develop
questionnaires for patient
use, utilizing a simple word
processor-like architecture.
Questions and Answers alike,
are entered by simple typing,
while
EKiosk©
converts these to an
interactive interface.
This runs the questions
interactively with the
patient, and stores the
resulting data in an SQL
database. Criteria may
be entered for form and
question navigation, and
audiovisual files may be
easily created and linked to
each question and answer so
that they are read to the
patient while they complete
it. While using the Form
Designer©, the question,
entire questionnaire or all of
the questionnaires may be
previewed in
EKiosk©,
so that the actual working
effect may be appreciated
during the design process.

Form Designer© may also be
used as an intuitive interface
for patient input of
demographic and medical
information. This may
lessen the burden of patient
data input, and the otherwise
necessary burden of collection
and verification of this data.
In this way, we may work
toward improved patient care
